In [[Aswan]], Egypt, a family arrives at their home to find that something is wrong. The parents go to the cellar of the house, which is revealed to be built on top of a mostly-buried black pyramid. One morning Katie is visited by the woman from Aswan, known as the Magician. She claims to be her friend Layla's mother, and lures Katie to the fence with candy before abducting her. Charlie chases them through a sandstorm but loses them. They report the kidnapping to the authorities, working with an English-speaking junior detective, Dalia Zaki, while the lead detective heavily suspects the parents.

The family settles Katie into her bedroom, refusing to seek further medical help. However, Katie's behavior is erratic and unpredictable and she frequently escapes her room and mutilates herself. Noticing the inscriptions on Katie's bandages, Charlie consults an archaeology professor at Albuquerque Community College. The professor explains the transcriptions are about the ancient demon Nasmaranian, the destroyer of families and communities which is kept at bay through a ritual mummification that binds the entity using script-laced wrappings embedded in skin. The more the scripture is removed, the more powerful the demon becomes.

During the chaos, Layla arrives home and gives Dalia a VHS tape with the label 'Katie'. Katie shed more of her bindings, takes control of her sibling's minds and later attacks and kills Carmen. At Carmen's funeral, Katie and Maud cause chaos by terrorizing the guests and desecrating their grandmother's corpse.

While Charlie and Larissa watch the tape, Katie hypnotizes Sebastian upstairs, forcing him to help her skin herself to remove more scripture. She attacks her parents and Dalia, beating Charlie bloody and forcing a scorpion into Dalia's throat. Having memorized the demonic transfer chant from the tape, Dalia helps the family transfer the demon from Katie into Charlie, who willingly sacrifices himself. Bound inside a coffin in the basement, Charlie manages to tap "I love you" in Morse code. Katie, now free of demonic possession, slowly starts to recover and build a proper relationship with her siblings.

At Carmen's funeral, Katie escapes her locked room, bites a hole in Carmen's corpse and drinks the embalming fluid. In a trance, Maud tears out her own teeth and begins attacking the others with Carmen's dentures.
