Since this is a list of boosters, shouldn't NG-003 count as a success (for the booster), apparently the 2nd failed to do a required 2nd burn to put payload in high enough or usable orbit. If we want to show it as failure perhaps change column heading to "Mission". Soon FAA may regard even uncontrolled 2nd stage reentry as worthy of stopping launches until investigation and agreed corrections.
